Title: Sworn oath form for notary

Hey Guys,

On the Korvia site in the diploma section it says I need a sworn oath form filled out by me. The form needs to say "the original document is real and authentic". Isn't that the point of a notary? Did you guys fill out a separate sworn oath form for your diploma? I would appreciate your help on this.

The form you want is generally known as a "Copy Certification by Document Custodian". You can use google to find a copy of the current form for your jurisdiction but the notary you go to will have it as well. You'll need to make a copy of your degree and bring the original with you for the Notary.
